The **highest urgency incident** — major user-facing outage or active data breach requiring immediate response.

Defining pager rules and executive notification thresholds.

#When not to

Labeling every failed unit test Sev-1 — reserve for customer-visible impact.

50% of `/api/chat` requests fail for 10+ minutes → page on-call and start incident channel.
